IMPORTANT INFORMATION
Parent Signature: Even if you register
on-line, the Ontario Lacrosse Association requires a signature by a
parent/guardian on the “OLA card” for each player. Please be ready to
attend one of our registration dates to verify and sign this form.
Proof of Birth: A copy of a “proof of age document (Birth certificate or
Passport) must be provided at time of registration for players who did not
participate in 2011. Please come prepared.
Payment Information: Cash and cheques are accepted at “in person” registration dates. Visa and
Mastercard are accepted in our on-line registration.
Non-resident players: If you do not currently
reside in Caledon-Vaughan boundaries, then you require a release to play
lacrosse for CVBLA. The release form can be obtained from the Ontario Lacrosse
Association website under “forms”. This form then needs to be completed and
signed by your home centre. Please bring the completed form to a registration
date.

MULTI-PLAYER DISCOUNT |

Bandits Lacrosse offers a family discount if 3 or more members of the same
family register for the same lacrosse program (e.g., field or box). 50% discount
on least expensive program will be applied. For additional information, please
contact our registrar. |

Pre-Season Clinics |
|
|
Pre-Season clinics will be held at ACTS Saturdays starting
Jan. 28 for 8 weeks.
· 1:30-2:30 for Peewee and older.
· 2:30-3:30 for Tyke and Novice.
A great deal for $60.
Great for those interested in some extra practice before
the season starts. New players and houseleague
players welcome in Tyke and Novice age groups.
Players must be registered for the
2012 season to attend these clinics. OLA cards must
also be signed by parent/guardian before players go on the
floor. |
